Working Papers (Employment Certificate)

All students between 14–17 years old must obtain an employment certificate to work (unless otherwise specified).

How to Apply:

Gather all necessary documentation (listed below) and submit copies in person to Ms. Salcedo Perez in Room 308.

Required Documents:

• Application Form (Section 1 completed and signed by a parent/guardian): AT-17 Application
  
• Birth Certificate (proof of age)
• Physical Form: CH205 Health Form 2025–26
• Doctor’s Note (stating the student is “fit for employment”): Physical Fitness Certification
